To enable us to assign a key worker best suited to your child and to prepare a personalised care plan to ensure your child has a safe and enjoyable time at ADDUP, we require further information than is given on the membership form, so please complete the form below, giving as much information as possible.
Please complete a form for each child, aged 5yrs or older, that you included on the membership form.
